The Advance Care Planning Respecting Choices© training program is a specialized course to prepare health
care professionals and community leaders to facilitate advance care planning discussions. This nationally
recognized program was developed by Gundersen Lutheran Medical Foundation (LaCrosse, Wisconsin) and is
endorsed by The Carolinas Center for Hospice and End of Life Care. The Respecting Choices© program takes
a comprehensive approach to train participants to facilitate advance care planning discussions, generate
dialogue about informed end of life care decisions, and work within their organizations to promote an
environment supporting these efforts. Individuals who become ACP facilitators will use the information,
ideas, methods and materials to:
- Facilitate advance care planning for individuals and small groups;
- Make presentations about advance care planning to community groups;
- Educate staff about advance care planning and the use of advance directives;
- Assist with the development and management of advance care planning systems and practices.
